Self Referral For Local Services

Did you know you can self refer for some services?  Please see details below. 

​CPFT Psychological Wellbeing Service

Offers free NHS talking therapies for those aged 17 and over registered with a GP in the area who are suffering from mild to moderate depression and anxiety disorders including generalised anxiety, social anxiety, post traumatic stress, health anxiety, panic, phobia or obsessive compulsive disorder.  This page also has links to several self-help documents.

Podiatry Service

You can self refer to this service for issues such as foot ulceration or infection, nail surgery, ingrowing toe nails and customised insoles or orthotics. 

Active Living

This exercise on referral scheme is for people whose GP feels they would benefit from a healthier and more active lifestyle.  Please pick up a referral form from reception.

NHS e-Referral Tracking

If your doctor has referred you to a specialist for further care you will be asked to book your appointment using the ‘NHS e-Referral’ system.

You will receive a letter from us within 2 weeks giving details of how to book your appointment and any passwords you may need.

To track your e-Referral you will need the following details to log into the e-Referral website:

  • Your date of birth
  • Reference number – this will be on the first page of your letter
  • Password – this will be on the second page of your letter
